Location: Ottumwa, IA

Hospital:

• 120+Bed Short-Term Acute Care Hospital
• Joint Commission-accredited facility
• Part of a larger healthcare system

  Director of Case Management Responsibilities:

  1. Lead, educate, and oversee the daily operations of Case Managers and Social Workers.
  2. Ensure compliance with regulatory standards by monitoring documentation within the Case Management Department.
  3. Spearhead the implementation of the hospital-wide case management program.
  4. Manage care facilitation, utilization management, case management, and discharge planning processes.
  5. Maintain an onsite presence as required.
  6. Directly report to the Chief Financial Officer (CFO).

  Director of Case Management Qualifications:

  1. Possess an Associate’s Degree in Nursing.
  2. Hold either an Iowa RN License or a Compact RN License.
  3. Have a minimum of three (3) years of experience in Hospital Case Management, including utilization management and discharge planning.

• Monitor Case Management Department’s documentation to ensure meets regulatory compliance.
• Implementation of the case management program at the hospital level
• Care facilitation, utilization management, case management and discharge planning.
• Required to be onsite.
• Reports to CFO

Director of Case Management Qualifications:

• Associate’s Degree in Nursing
• Iowa RN License or Compact RN License
• Minimum Three (3) years of Hospital Case Management experience in utilization management, case management, discharge planning experience
